Let’s take a look at the patient’s chart in athena after someone uses the ACP tool.
In the“Quality” tab, the Advance Care Planning measure no longer shows as “past due.”
In the “Find” tab under “clinical documents,” at the top see an “acknowledgement of advance directive” document, which is a carbon copy of the discussion notes.
In the encounter, the discussion notes have been copied into the ACP textbox.
Lastly, on the billing tab the billing code is automatically posted whenever we save the discussion. In addition, the modifier of 33 is automatically appended to that code. The 33 modifier signifies that it is part of the covered Medicare Annual Wellness visit, This modifier can waive the patient’s co-pay, deductable, and co insurance so that there is no cost sharing Modifier 33 – Preventive Services usage and guideline policy | Medical Billing and Coding - Procedure code, ICD CODE. We also add a diagnosis that ties this to Advance Care Planning, and it allows us to append the billing slip with that note.
