# Product Update: Advance Care Planning Enhancements (May 2025)

## We’re releasing a series of updates to support and scale Advance Care Planning (ACP) for patients with serious illness, including: - Appointment note notifications - MSSP & ACP flags within the patient chart - Documentation workflows

- ### **Appointment Note Text**

When a patient is flagged for ACP due to serious illness, the following will appear at the **bottom of their appointment notes**:

“An Advance Care Planning discussion is needed as soon as possible because the patient has a serious illness.”

- ### **Chart Labels for MSSP and Serious Illness Patients**

We’ve introduced chart labels that help teams quickly identify ACP-eligible patients using
