# Manage Advance Directives

Providers can use the ACP Tool to print new Advance Directives and view completed Advance Directives that have been uploaded to athenaClinicals.

**View Completed Advance Directives**

Documents that have been completed and uploaded to athenaClinicals or the ACP Tool appear in the Advance Directive tab under the Completed header.

**Create New Advance Directives**

Depending on the document, new Advance Directives can be completed through the following methods:

- Print a blank form
- Print with a cover sheet
- Completed digitally in-visit (with eSignature)
- Email

To navigate to this menu, click the blue "Advance Directive" tab and click the "New" header.

**Upload Completed Advance Directives**

Upload completed Advance Directive documents to athenaClinicals with one of the following document labels:

- FIVE WISHES
- HEALTHCARE PROXY
- LIVING WILL
- DO NOT RESUSCITATE (DNR)
- PHYSICIAN ORDERS FOR SCOPE OF TREATMENT (POST)
