# How does the Huddle determine if a patient is eligible for Transitional Care Management?

An eligible for Transitional Care Management flag will display if the following criteria is met:

1. The patient has recently been discharged from a hospital setting
2. A follow up call to the patient has been made and a patient case has been recorded
3. The patient is coming into your practice within the 14 day eligibility window
