# Access the ACP Tool

The ACP Tool directly integrates with athenaClinicals®, so Providers can access it directly from the Patient Chart.

From the Patient Chart, use the following navigation:

“Hamburger Menu” at the top right corner \> Third party applications \> Advance Care Planning Tool. 

If you are a first time ACP Tool user, the tool will automatically setup an account for you, so no additional steps are necessary.
