Advance Care Planning (ACP) is an ongoing process for all adults where a person shares values and goals for future healthcare decisions, if they can no longer communicate them due to injury or illness.
- A conversation that is voluntary, with an opportunity to decline.
- A conversation specific to a patient’s health status and medical condition.
- A face-to-face (in-person or telehealth) conversation with the patient or their healthcare surrogate.
- A conversation including the explanation and discussion of advance directives (naming a healthcare surrogate, wishes for future care) NOTE: Advance Directive forms do not have to be completed to bill for the service.
